Our membership is available year-round, so you can always sign up during your next visit with us. Your membership adjusts with your treatment plan, so you are never locked in to one type of service.
How do I pay for my services?
Let’s say you commit to getting $300 worth of Dysport every 3 months. Normally, you would pay FVPS $300 upon checkout. With the Renaissance Club, you pay nothing directly to FVPS. You would pay $100 that day on the HintMD app, and then you would be charged $100 monthly thereafter. You would then come into the office every three months to get your Dysport. It actually amounts to only $95 per month for 3 months because Renaissance Club members pay less for Dysport. So, would you prefer to pay $300 all at once or $95 per month for three months?
It’s as simple as that. There are no gimmicks or hidden charges. With the rewards of being a subscriber, your true cost of services is even lower. Even for people who are used to paying as they go, this plan is too good to pass up.
The membership program is completely free to join. There are no enrollment fees. Members are paying only for their treatment plan. There are no extra monthly fees. To enroll, the minimum service levels are at least one of these:
Can I cancel my membership?
Members can adjust their program at any time. Members may cancel without any cancellation charges or penalties by notifying FVPS. If members cancel before they have paid off the services received, they simply pay the balance due on their account. FVPS may cancel accounts which fall below the minimum service levels.
What if I am charged for a few months, but missed my appointment?
You can always come in and receive the treatment at a later time. The money is banked for you as a credit on your account. If you elect to not receive the treatment, we will refund you for the payments you made.
Some members may choose to pay ahead and have a credit on their account until they get their treatments. This is very smart, because they get the preferred RC member prices while never owing money.
I change or tweak my service every time I come here. Does the Renaissance Club work for me?
Patients are never paying for services that they don’t receive or for things that don’t accurately reflect the treatment plan. Treatment plans are flexible, so that patients can increase or decrease the amount or frequency of their services as long as they meet the minimum service levels listed above.
When the treatment plan changes, the patient is charged that day for any balance from the previous treatment plan. For example, if you signed up for Dysport every 4 months, and come in again at 3.5 months, you would then be charged by HintMD for the balance of your first visit before making payments on this next visit.
How do I sign up, and how long does it take to enroll?
The Renaissance Club is hosted by HintMD, a cloud-based program. Signing up for the Renaissance Club takes 10-15 minutes. Your time spent enrolling will be quickly offset by the reward you get that day. FVPS will add you as a new member, create your personalized treatment plan, and add your credit card information.
Your first payment will be due immediately for the service you received that day. A receipt and instructions to download the app will be emailed to you. You will be automatically billed in subsequent months according to your treatment plan.
Whom do I contact if I have a question regarding my monthly payments?
Call the office at 920-233-1540 if you have questions about your Renaissance Club balance. Some inquiries require research with HintMD customer service, so those inquiries may take up to 2 business days to answer.
What happens when I come back for a follow up visit?
Identify yourself as a Renaissance Club member, so that the office can mark which services were received. The office does not need to run your credit card on future visits. All charges are run through the HintMD portal. Any charges that were not part of the treatment plan, are billed separately and are due that day to FVPS.
If changes were made to the plan, such as added services or changed frequency, the office will edit your plan, and make the appropriate changes to check out.
